What is the difference between ordinary sadness and depression?

Sadness has a clear trigger, is proportionate to the event, eases with time and leaves your capacity to enjoy other things intact. Depression is persistent (more than 2 weeks), affects sleep, appetite and energy, and includes anhedonia — the loss of pleasure in activities that used to be enjoyable.

Does therapy work without antidepressants?

For mild to moderate depression, CBT or IPT alone are effective — 50-60% remission after 16-20 sessions. For severe forms, therapy plus medication is the gold standard and reaches 60-65% remission. The medication decision belongs to a psychiatrist, after assessment.

Is depression genetic?

There is a genetic component — roughly 40% higher risk if a parent had depression. But genetics is not destiny. Environmental factors (stress, trauma, isolation, sleep) determine whether the predisposition ever shows. Many people with a family history never develop depression.

How quickly do antidepressants start working?

The full therapeutic effect appears 4-6 weeks after reaching the therapeutic dose. The first 1-2 weeks can intensify anxiety — a known effect, monitored by the psychiatrist. Do not stop treatment on your own if side effects appear; talk to your doctor.

Can I do therapy for depression online?

Yes, for mild to moderate forms. Studies show efficacy equivalent to in-office work. For severe forms with active suicidal ideation, being physically present is often preferable. The therapist assesses this in the first session and recommends the right format.
